Registration is a two-step process:

Registration, Part 1

Call the Central Registrar at 356-8222 to make an appointment. When you come for your appointment, please bring a completed New Student Registration Form and the following:

Proof of Residency:

ONE of the following items:

  • If renter, current lease, which includes landlord’s name, address and telephone number as we may need to contact them to verify the contents of the lease.
  • If renter without a current lease, please provide current rent receipt with landlord’s name, address and telephone number as we will need to call and speak with them.
  • If homeowner, current school taxes for Mohonasen CSD, deed, and/or mortgage statement
  • If you have just had your closing, closing papers including all realtor information.
  • If moving in with someone else, that individual must provide one of the documents listed above as well as be present at the time of registration to sign an affidavit.

AND

License and car registration with name and current address.

AND at least ONE of the following items:

  • Utility bills(s) with current name and address (we prefer two of these, if possible)
  • License and car registration with current name and address
  • Current hospital or health records
  • Band and/or credit union account statement with current name and address
  • Voter registration card with current name and address
  • Income tax forms
  • Home and/or renter’s insurance
  • Change of address form from the Post Office
  • State or other government issued identification
  • Documents issued by federal, state or local agencies

Proof of Age

Please provide ONE of the following:

  • Child’s birth certificate with seal (not hospital) or certified copy
  • Baptismal record
  • Passport
  • Acceptable Alternative Forms: If you are not able to provide any of the forms listed above, the district will consider the following documents:
    • Official driver’s license (of student)
    • State or other government issued identification
    • School photo identification with date of birth
    • Consulate identification card
    • Hospital or health records
    • Military dependent identification card
    • Documents issued by federal, state, or local agencies
    • Court orders or other court-issued documents
    • Native American tribal documents
    • Records from non-profit international aid agencies or voluntary agencies

Other Mandatory Student Information

  • Immunization Record
  • Custody papers, if applicable. If student is not your biological child, you must present official court-generated documentation that proves a permanent and total transfer of custody and control of the student to you.
  • Child’s Individual Education Program (IEP), if applicable
  • Child’s 504 Plan, if applicable
  • Child’s last report card or transcript

When you have everything you need to register, call our registration office at 518-356-8222 for an appointment . If you fall under any of the special circumstances on the registration home page, please also call 518-356-8222. Upon registering your child(ren), the district will provide you with additional forms to be completed.

When you have finished at the District level, you will receive a registration packet to bring with you to the building level for Part Two.

Registration, Part 2

When you register at the building level, please bring:

  • Court papers, if granted custody
  • Transcript and last report card
  • Current school schedule
  • Original Immunization Forms
  • Physical Forms, signed by the physician
  • Dental Health Certificate, signed by the dentist
  • IEP(Individual Education Plan) for special education services
  • 504 plan, if applicable

You will also need to fill out:

  • Health History cards
  • Emergency Cards
  • Release of information forms
  • Requested forms vary by building
