Important: Learning Mode Change Form Deadline is Tuesday, Oct. 27 

Please complete this Learning Mode Change Form ONLY if you are CHANGING learning options for Quarter 2. Please only complete the form once for each student in your household and ONLY if that student is CHANGING learning options.

For example, if your child is a Completely Remote Learner (CRL) and you would like your child to return to school in-person in the hybrid plan, complete the form. If you child is a CRL and you are continuing in the CRL plan, do NOT complete this form.

CRL: Completely Remote Learners attend all classes virtually on all days. They do not receive Mohonasen transportation, and they do not attend classes in-person.

Hybrid: Hybrid learners attend school in-person, two days at a time, on a rotating basis.

Please note: A change from CRL to hybrid may require changing class sections and teachers

Remember: The deadline to submit this form is Tuesday, Oct. 27.
