Registration is a two step process:

Part One...

The District level:

Be sure to bring with you the completed registration packet:
a. Is the Student Registration Form complete?
b. Do you have an original Birth Certificate with the court seal?
c. Do you have primary physical custody of the child(ren)? Be prepared to show custody agreement.
d. Do you reside in the District? Be prepared to show 3 proofs of residency.
Original Lease or Original contract to build or buy a house or Original Deed or Tax document from the Rotterdam Town Hall.
PLUS (Two of the following)
  - National Grid bill with current name and address
  - License and car registration with current name and address
  - Certificate of Occupancy issued by town/village
  - Automobile insurance with current name and address
  - Bank and/or credit Union Account Statement with current name and address
  - Voter Registration Card with current name and address
  - Telephone bill with current name and address
  - Home and or renter’s insurance with current name and address
e. Has your child been properly immunized? An original immunization form, signed by a physician, must be presented. Building level will verify.
f. Have you completed the Home Language Questionnaire, if applicable?
g. Have you completed the McKinney Vento Questionnaire, if applicable?

Other documents needed, if applicable:
- Release of Information Form (provided by district)
- DSS Benefit Certificate

Part Two...

The Building level:

What do I bring to the building level?
a. Court papers, if granted custody
b. Transcript and last report card
c. Current school schedule
d. Original Immunization Forms
e. Physical Forms, signed by the physician
f. Dental Health Certificate, signed by the dentist
g. IEP(Individual Education Plan) for special education services
h. 504 plan, if applicable

What else will I need to fill out at the building level?
a. Health History cards
b. Emergency Cards
c. Release of information forms
d. Requested forms vary by building
